System of Care
The Greater Norwalk System of Care is a process that supports families, with children who are experiencing difficult and complicated emotional and behavioral needs, to form a team of people that are committed to help improve the family's situation and maximize the child's functioning at home, in school and in the community.
The family's team, called the Child and Family Team, is made up of people the family trusts will work with them to create a unique plan they can use in achieving their goals and vision.
A Care Coordinator is assigned to the family to assist in identifying their needs, developing the Child and Family Team members and coach the family to guide the team in making a plan that is suitable for them using both traditional and non-traditional services.
Program Eligibility:
The program is available to any parent(s) with a child or adolescent up to the age 18 (or 21 if still in school) who require coordination of services due to complex emotional and behavioral needs in the Greater Norwalk Area (Norwalk, Weston, Westport, and Wilton).
Accessing Services:
The services may be accessed Monday through Friday from 9:00 am to 5:00 pm by calling (203) 299-1315 extension 109.
Client Fees:
There is no fee to the family for participating in Care Coordination. This is a free service to those who meet the criteria.
